Community Support Coordinator - Prescription Plus

Prescription Plus is a social prescribing project in Crawley which improves the health and wellbeing of local people by supporting them to connect with community support and activities.

We are recruiting for a Community Support Co-ordinator to join the Prescription Plus Team. You will be linked to four GP surgeries in Crawley and responsible for taking referrals, meeting and assessing patients, and supporting them to make positive changes to their lives by linking in with a range of community activities. You will be part of a small team developing and expanding this project in Crawley.

Due to the nature of this role, it is essential that you have a driving licence and access to your own vehicle.

35 hours per week (37fte)
Fixed-term contract until 30th September 2020

Senior Community Link Worker / Development Officer - Age UK West Sussex

35 hours a week based in Mid-Sussex

The Senior Community Link Worker / Development Officer will work alongside existing services to bridge the gap between GP Surgeries, other health and social care agencies and local community services. They will support patients to navigate their way around Health, Social Care and Voluntary Sector Services to access community based solutions to help manage their conditions and improve their quality of life.

They will work with specialists to organise, set up and deliver group therapies and peer support groups to provide patients with specialist and peer support to achieve their desired outcomes.

The CLSDO will work with a team of volunteers to provide patients with the information and advice to identify solutions to their circumstances and bring about the changes necessary to improve their health, wellbeing and quality of life.

The other part of the role will be to manage, supervise and support the Community Link Workers (CLW) and volunteers.

The post requires you to have a full driving licence and use of a car.

Home Helps - Age UK West Sussex

Age UK West Sussex require SELF EMPLOYED HOME HELPS in CRAWLEY, MID SUSSEX, CHICHESTER, ARUN AND ADUR to help with household tasks, cleaning, shopping and provide social support to assist older people to remain independent in their own homes.

Working between Monday and Friday, the hours are flexible and according to your availability.

Car drivers preferred but not essential.

The Complex Needs Worker will work alongside refuge staff to provide a safe and empowering environment for women escaping domestic abuse. She will be responsible for implementing a range of interventions and support measure as part of an integrated package of healthy, therapeutic and social support to women experiencing domestic abuse with additional complex needs.

Cafe Assistant - Groundwork South

We are looking to recruit weekend staff to work in our busy Beautiful Café bar in Tilgate Park.

This is an exciting, flexible opportunity for bubbly enthusiastic people who have experience in either catering / waitressing/food running or Bar work although full training will be provided.

You will be responsible for assisting the Café manager with café operations, which includes preparing food and drink, ensuring café is well stocked and providing a safe and clean environment for both staff and customers.

Working within your team, you will assist in marketing the café, providing members of the public with information about the Centre and ensuring café is provided for during event programmes.
